Step 1: Organization Information

First , lets collect some general information about your organization.

Answer "yes " if any products you purchase are for export to any other country. This includes where we are delivering overseas on your behalf or where we deliver to you locally and you then export
Answer “yes” if you buy our products and then re-sell them to customers. Note – we will still classify you as a reseller for these purposes even if you are acting as a non-profit / charitable organization or as a Non-Governmental Organizations (NGO’s).
Answer yes if your activities include incorporating products purchased from Fisher into your own products (as part of a kit e.g. centrifuge tube sold within a Centrifuge bundle or as a component e.g. tubing incorporated into an instrument), which will then be marketed and sold under your own brand name?
Answer “yes” if you are making payment for goods purchased on behalf of another organization or an end user that does not belong to your organization, even if the goods are being shipped direct to the end user.
